Privacy and Legal Statement for Elevate Your Health Colorado Web site, a Kaiser Permanente Health Care Initiative.

The Elevate Your Health Colorado Web site is a service provided by a group of four contracting parties, herein referred to as "Cooperative" or "We." The parties are Kaiser Permanente Colorado, Zizzo Group, Inc., The Denver Newspaper Agency, and KCNC CBS 4. This Web site, offers health information for the benefit of individuals agreeing to the "Conditions of Use" as defined in this Privacy and Legal Statement.

We are committed to protecting the privacy of the users of this Web site. We will use and disclose the information you provide us as stated in our Web Site Privacy Statement.

In addition, if you provide health information that identifies who you are (as when health information also includes your name) we will only use and disclose such information as stated in the Notice of Privacy Practices. The Notice of Privacy Practices is different for each of the geographic regions we serve.

Web Site Privacy Statement
We respect your desire to protect your privacy. We are pleased to provide online access to information regarding health care topics and services in a way that does not require you to reveal any personal information. Each time you visit our site, information about your use of the site is collected automatically by using standard cookie and web server log technologies. Our use of these standard technologies is explained below. Our use of cookies and web server logs for this site is such that no personally identifiable information is collected about you or any other users of this site.

Revisions to the Privacy Statement
We may revise this Privacy Statement from time to time as we add new features or as laws change that may affect our services. If we make material changes to our Privacy Statement, we will post notice of this on our Web site. Web site Visitor Data
The Cooperative routinely gathers data on Web site activity, such as how many people visit the site, the pages they visit, where they come from, how long they stay on the site, etc. The data is collected on an aggregate, anonymous basis, which means that no personally identifiable information is associated with the data. This data helps us improve site content and overall usage. The information is not shared with other organizations for their independent use.

Collecting and using personal information
Except as disclosed in this Privacy Statement, we do not collect any identifiable information about visitors to this site. The policies, sources, and uses of information are outlined in Sections 1 through 13 that follow:

1. Web logs
This site uses standard Web server logs to automatically gather statistics about the visits to this site. Web server logs do not contain any personally identifiable information about visitors to this site. Information may include the name of the domain from which a user accesses the Internet (such as aol.com, abc.org, etc.), the IP address which is automatically assigned to your computer when you get on the Internet (a static IP address may be identifiable as being connected to you, while a dynamic address is usually not identifiable), the type of browser and operating system you use, the date and time the user accesses our site, the pages the user views, and how much time the user spends at our site. If a user links to our site directly from another Web site, the Internet address of the Web site from which the user links to our site may be recorded. Similarly, if a user links to another site from our site, that information may be collected.

All Web logs are stored securely, and may only be accessed by the Cooperative on a need-to-know basis for a specific purpose. ElevateYourHealthCO.com uses Web log information to help us design the site, to identify popular features, to resolve user, hardware, and software problems, and to make the site more useful to visitors.

2. Internet cookies
This site utilizes a standard technology called "cookies" to help manage the user's browsing experience on the site. A cookie is an alphanumeric file that transfers to the user's computer during a session to: (1) to keep track of the pages previously selected during the session, in order to enable "Back to" capability; and (2) to enable general usability statistics that can be used to make the site better. Cookies are incapable of identifying the user. However, this Service does not control the use of cookies used or linked by contracted or third party services or Web sites.
You may elect to be notified of cookie placements or elect not to receive cookies by adjusting your browser settings. Even if you elect not to receive cookies, you will still be able to navigate this Web site and use its features and services.

3. E-mails
We will manage any e-mail communications with you for the Elevate Your Health Colorado program. The e-mail addresses of those we wish them to contact are never used for any purpose other than to communicate with you on our behalf. We will never shares any information with anyone else.

Even if you have given us permission to send e-mails to you, you may revoke that permission at any time by following the "unsubscribe" information at the bottom of the e-mail.

4. Evaluation and research
We will periodically ask users to complete surveys asking about their experiences with features of the Web site. Our surveys ask visitors for demographic information such as age, gender, and education, but will not request that users provide specific information about any medical condition. We use survey information for research and quality improvement purposes, including helping The Cooperative improve information and services offered through the Elevate Your Health Colorado program. In addition, users giving feedback may be individually contacted for follow-up due to concerns raised during the course of such evaluation. Demographic information and Web log data may be stored for future research and evaluation.

5. Children
We do not knowingly collect personally identifiable information from users under the age of 13 on the ElevateYourHealthCO.com Web site.

6. Disclosures
We may disclose personal information to any person performing audit, legal, operational, or other services for us. We will use information which does not identify the individual for these activities whenever feasible. Information disclosed to vendors or contractors for operational purposes may not be re-disclosed to others by such a vendor or contractor.

We may disclose personal information when required to do so by a subpoena, court order, or search warrant. We may disclose personal information as we deem it appropriate to protect the safety of an individual or for an investigation related to public safety or to report an activity that appears to be in violation of law. We may disclose personal information to protect the security and reliability of this site and to take precautions against liability.

7. Opt out
If a user makes a request to receive information in an ongoing manner through this Web site by providing their e-mail address (for example, requesting to be added to an e-newsletter distribution), a user may make a request to discontinue future mailings. Similarly, if you receive information about a service offered through the Elevate Your Health Colorado program through e-mail, you may make a request to discontinue receiving similar messages in the future. All such materials sent to you by e-mail will contain information about how to opt out.

8. Other requests to limit use and disclosure of your personal information
State and federal laws may allow you to request that we limit our uses and disclosures of your personal information for the purposes of research and quality improvements of the site. However, by law, we do not have to agree to your request. It is our statement not to agree to requests for restrictions beyond what is described in the "Opt out" section above.
